We regularly advise our clients who suspect abuse and neglect in a Virginia nursing home to file a complaint with the state. These investigations are usually prompt, without cost to the family, and the results are reported to the individual(s) who filed the complaint.

We decided to share this advice to the readers of this blog. You may make a complaint to the Virginia Department of Health’s Office of Licensure and Certification in writing, by telephone, or fax using the following contact information:

Complaint Intake

Office of Licensure and Certification

Virginia Department of Health

9960 Mayland Drive, Suite 401

Richmond, Virginia 2323-1463

Telephone: 800-955-1819 (Toll-free) Fax: 804-527-4503

Web site: http://www.vdh.virginia.gov/OLC/complaint

Specific information about the date, day of the week, and time of day when the problem was observed, as well as the names of the individuals involved, will assist investigators. Reports can be made anonymously, but if you file it anonymously you will not be able to find about the result of the report, so it is a good idea to leave this basic information so that you will know the progress of your report. Your identity is protected by law, so there is no need to fear any about giving your personal information.
